To provide a comprehensive menu of Non-Institution, Post-Acute, care and services to the community. To provide person centered care in a therapeutic, community focused environment. De-Institutionalize Nursing Facility residents who are at a Light or Moderate Level of Care. To prevent premature clinical (re)institutionalization. To enable clients to remain a viable individual, active family member, and invaluable community member regardless of their age, religion, disability, infirmity, sex or sexual orientation, or other such civil rights frequently discriminated against. To enable citizens to define and determine their quality of life and receive the quality of care and services that meet their values, needs and resources.
Prevent Premature Hospitalization or Nursing Home Placement. Facilitate timely deinstitutionalization. Deinstitutionalize those who have been prematurely or continue to be inappropriately (re)institutionalized-hospital or nursing home. Assist Community Members to attain the highest level of independence in the most practical and cost efficient manner and setting. Provide a mechanism for the community (e.g. other organizations, institutions, and businesses), family, and the client them self to actively participate and provide services/resources/information/ supplies/equipment/and care to assist our aged, infirmed and/or disabled. Coordinate ancillary services as required e.g. rehabilitation, dialysis, laboratory radiological services, and durable medical equipment etc. Insure the provision of the required care to individuals or groups in the most appropriate yet least restrictive environment that is clinically, psychosocially, and culturally appropriate and desired. Deliver quality care and/or services, in a timely and compassionate manner which represents quality of life as defined by the client and the home community at large. Provide a continuum of status post institution care and services. Provide a menu of services in a variety of settings based on community and client needs. Develop the means and ways to keep the client integrated within their family and community. Help save the State Medicaid program so that funds will appropriately exist where absolutely needed.
